Be aware that with the proper referral your insurance usually covers some if not all of the consultation costs.

You or your insurance will be responsible for the costs of labs and studies NOT done as part of your hospital stay.

All patients for both the LapBand and Bypass require a psychological and a pulmonary/critical care clearance.  Select patients require additional work-ups pre-operatively, such as gastroenterology and/or cardiology. My office helps to facilitate these evaluations. They are for YOUR safety. These services are almost always covered by your insurance, even if you are a self-financed patient. Clearly, if not a covered benefit then you will have to make arrangements to cover these costs.

Financing and loan considerations are available through Dr. Snyder’s office through an outside agencies that have a special interest in helping with bariatric surgery candidates.

Rose Hospital and ASC have been generous in their billing assessments for these procedures. In the event of a complication or a hospital stay beyond 5 days for gastric bypass and 2 days with LapBand, your insurance will be billed and considerations will be weighed on a case-by-case basis. We have administration’s guarantee that our patients will not be financially punished by costs beyond their control within the realm of the reasonable amount of risk and uncertainty surrounding any major surgical procedure. However, the patient bears the risk of potential additional charges beyond the established global period.

These charges are established as of May 1, 2007. They are subject to change and negotiation. Please check with Dr. Snyder’s office concerning your personal charge considerations prior to scheduling your surgery.

All payments must be received 7 days prior to the date of your scheduled surgery. All payments received within 14 days of your planned surgery must be in the form of cash, credit card or a cashiers’/certified check.

All payments for hospital services, anesthesia services, and consultants must be paid prior to the date of service. Camera operator services will be billed separately.

Plan for a possible set of minor charges with a maximum total of $200. This will ensure no “surprise” incidental charges.

As always, please call us if you have any questions or concerns.

Very Truly Yours,

Michael A. Snyder, MD, FACS, PC
Stacie E. Perlman, MD
